Semaglutide: The Deepest Trial Record on This Desk

A GLP-1 receptor agonist with landmark trial data across diabetes, weight, heart, and kidney outcomes — and the compound that lost its own head-to-head matchup.

The short version

Semaglutide is a 31-amino-acid peptide engineered to mimic and outlast a natural gut hormone, GLP-1, which the body normally clears within about two minutes. Two structural changes stretch that lifespan to roughly a week, making once-weekly dosing possible: an artificial amino acid that blocks the enzyme that would otherwise cut the chain apart, and a fat-like side chain that lets the peptide cling to a blood protein instead of being filtered out by the kidneys.

Semaglutide is FDA-approved for type 2 diabetes, chronic weight management, cardiovascular risk reduction, and, since 2025, a liver condition called MASH, backed by some of the largest trial programs of any drug in this class. This page reports those numbers as they were measured, and reports just as plainly that semaglutide was directly outperformed by tirzepatide in the one trial that compared them head to head.

What it is

Semaglutide shares about 94% of its amino-acid sequence with natural human GLP-1. Two substitutions do the structural work: at position 8, a non-standard amino acid (Aib) blocks the enzyme DPP-4 from cleaving the chain, and at position 34, arginine replaces lysine. A separate lysine at position 26 carries a C18 fatty-acid side chain via a small linker, which binds reversibly to albumin in the blood, shielding the peptide from kidney clearance and stretching its half-life to about a week. The oral tablet formulation adds an absorption enhancer (SNAC) that briefly changes stomach-lining chemistry to let a fraction of the dose cross intact; oral bioavailability remains under 1%, a concrete illustration of how much harder peptides are to deliver by mouth than an ordinary small-molecule pill.

How it works

Semaglutide activates GLP-1 receptors throughout the body. In the pancreas, it boosts insulin release only when blood sugar is already elevated and suppresses inappropriate glucagon release — a glucose-dependent mechanism that limits, but does not eliminate, hypoglycemia risk. It slows stomach emptying, prolonging fullness and contributing to nausea as a side effect. Its weight effect is largely central: the peptide reaches hypothalamic and brainstem circuits that govern hunger, activating fullness-signaling neurons and suppressing hunger-signaling ones, which is the mechanistic basis for the reduced food preoccupation many users describe.

What the research shows

Head-to-head against tirzepatide. In a 72-week, 751-person trial that gave each drug at its maximum tolerated dose, tirzepatide produced significantly greater weight loss than semaglutide — -20.2% versus -13.7% (P<0.001) [13]. This is the most direct comparison available, and it places semaglutide as the benchmark a newer dual agonist has since surpassed, not as the category leader.

Kidney outcomes. In 3,533 adults with type 2 diabetes and chronic kidney disease, once-weekly semaglutide reduced the composite risk of kidney failure, substantial eGFR decline, or kidney- or cardiovascular-related death by 24% versus placebo (HR 0.76) [14].

Cardiovascular outcomes. In 17,604 adults with established cardiovascular disease and overweight or obesity but no diabetes, semaglutide reduced major adverse cardiovascular events by 20% versus placebo (HR 0.80, P<0.001) [15] — a large, well-powered outcome trial, one of the strongest single pieces of evidence anywhere on this desk.

Weight management. In 1,961 adults with overweight or obesity, semaglutide 2.4 mg produced -14.9% mean body-weight change at 68 weeks versus -2.4% with placebo [16].

Safety review. A dedicated review characterizes semaglutide's overall risk-benefit profile as favorable in type 2 diabetes, with mostly mild-to-moderate gastrointestinal effects (nausea in roughly a third of patients), an increased risk of gallbladder disease, and pancreatic- and thyroid-cancer signals for which the review says definitive conclusions cannot yet be drawn given how rarely they occur [17].

Even here, on this desk's strongest evidence base, that review-level honesty matters: "cannot yet be drawn" is not the same statement as "ruled out."

Commonly described adverse effects: nausea is the leading complaint, often described alongside sulfur-smelling burps, bowel changes (constipation and diarrhea, sometimes alternating), acid reflux, and early fatigue on injection day. A smaller group reports hair shedding after months of rapid weight loss.

Cited cautions from the clinical literature:

  • Gastrointestinal intolerance: the dominant adverse-effect category and leading cause of discontinuation, concentrated during dose escalation [17].
  • Thyroid C-cell tumors / MEN-2 (boxed warning): based on rodent data; the reviewed literature notes the human signal is not established, but a personal or family history of medullary thyroid carcinoma or MEN-2 remains a contraindication on the strength of the animal finding [17].
  • Acute pancreatitis: a recognized class warning; the safety review notes that pancreatic-cancer signals specifically remain unconfirmed given how rarely they occur [17].
  • Gallbladder and biliary disease: an increased risk versus placebo, attributed largely to the pace of weight loss rather than direct toxicity [17].
  • Diabetic retinopathy with rapid glycemic correction: a documented trial-level signal in patients with pre-existing retinopathy undergoing fast HbA1c lowering [17].
  • Lean-mass loss, weight regain after stopping, hair shedding, and a pregnancy contraindication are all documented concerns in the broader semaglutide literature; this desk notes them as real, but without a citation meeting this hub's own reference bar in hand, does not attach a specific figure to them here.
